On a `no_results` result there is no `resultId` — do not call `render_doctor_results`; instead follow the narration to broaden the search. Insurance details materially improve the quality of the match: `insurancePlanId` (a specific plan id the user has confirmed) filters to doctors reported to accept that exact plan; `insuranceCarrier` is a broader fallback used when only the carrier name is known. With both fields unset, results are unfiltered by insurance and may include providers who do not accept the user's insurance.

Use this when the user has responded with any health insurance information (carrier, plan name, or both). Returns up to 10 ranked candidate plans matching the user's words.
